The nuclear industry has successfully integrated a lesson's learned mentality in the corrective action process. The principles of Root Cause Analysis (RCA) are a foundation of this philosophy. Many industry guidelines exist in this area including: INPO Good Practice OE-907 (INPO 90-004) Root Cause Analysis and NUREG/CR-5455 "Development of the NRC's Human Performance Investigative Process."

Altran Solutions has extensive experience in the processes of root-cause determination and apparent cause determination both inside and outside the nuclear industry. We frequently are an integral part of either a plant an integrated team or serve as an independent investigator.
