Single Point Vulnerability
As plants age, components previously considered as inherently reliable, begin to fail, often bringing the facility off-line. Often, these components are associated with secondary plant systems that do not have a significant level of component redundancy. A single point vulnerability (SPV) project is designed to identify these, often hidden, components. Once identified, SPV's can then be prioritized based on their impact on safety, plant operability and cost of risk mitigation. The risk of failure of high priority SPV's can be mitigated by application of preventive or predictive maintenance tasks, the application of training or by a design modification.
